The Consultation: A Shape Problem, Not Just a Size Problem

This patient came to the New Jersey Breast Reduction Center with a very particular set of concerns. Her breasts were not only sagging, but they were also tuberous in shape, meaning they had a narrow base, prominent projection in the lower portion, and enlarged areolae sitting low on the breast mound.

This wasn’t just about size. This was about architecture. Her breast shape needed a complete redesign.

She wanted her breasts to feel more natural, balanced, and lifted. And more than anything, she wanted the shape and areolae to look normal and proportional.

My Surgical Plan: Rebuilding from the Foundation Up

This case was about precision.

A small reduction alone would not solve the issue. She didn’t have overly large breasts. She had misshapen, sagging breasts that needed to be reshaped from the base upward.

During surgery, I focused on three core goals.

First, I removed a small amount of tissue to reduce volume just enough to create better proportion and symmetry.

Second, I performed a breast lift to raise the entire breast mound higher on the chest wall. This eliminated the drooping effect and brought the breast into a more natural position.

Third, I reduced the areola diameter significantly. This is essential in tuberous cases where the areola stretches disproportionately over time. By tightening and repositioning this area, the entire breast takes on a cleaner, more balanced look.

The Before: A Closer Look

In her before photos, you can clearly see:

  • The tubular shape with most of the fullness sitting low
  • Downward-pointing nipples located at the bottom of the breast mound
  • Very large areolae that visually dominated the breast
  • Loose, stretched lower pole skin

These are classic hallmarks of tuberous breast deformity combined with sagging (ptosis). And all of them needed to be addressed together.

The After: A Natural, Feminine Contour

Her after photos speak for themselves.

The breast mound is now rounded, compact, and sits higher on the chest. The nipple position has been elevated to the center of the breast mound, facing forward rather than downward. The areolae are smaller and balanced in proportion to the new breast shape.

Perhaps most importantly, the tuberous look is gone. Her breasts now have a natural contour with gentle fullness in both the upper and lower portions, exactly what a balanced breast should look like.

Her scars are healing beautifully and are strategically placed around the areola and vertically beneath the breast. These will continue to fade with time, becoming less noticeable as the months go by.

Final Thoughts

Cases like this highlight why breast surgery is never one-size-fits-all. This wasn’t about reducing dramatically. It wasn’t just about lifting. It was about reshaping, building a new foundation for the breast, and restoring the kind of balance and proportion that looks and feels completely natural.
